The information to be provided in the form includes the following:

1)    Payer’s company particulars

2)    Recipient’s particulars (resident / non-resident)

3)    Particulars of incentive payment

a)    Value of monetary incentive

b)    Value of non-monetary incentive

For year 2012, the payer company is required to submit Form CP58 [2011] to an agent, distributor or distributor no later than 31 May 2012.
